How do I make an appointment?

To schedule an appointment, please call 763-427-1311. Our staff will be pleased to schedule your appointment.

Do I need a referral to schedule an appointment?

Although our office receives patients that other dentists refer, it is unnecessary to have a referral to schedule an appointment.

Where are you located?

502 Jefferson Highway N., Champlin MN 55316. We are located on the corner of Highway 169 and Dean Avenue in the Champlin Services Center. Highway 169 is Jefferson Highway at this point in Champlin. Get Directions

What if I need to cancel an appointment?

We see patients by appointment only. We make every effort to be on time for your appointment and ask that you extend the same courtesy to our office. If you cannot keep an appointment, please give our office a 24 hour (1 business day) advance notice. We reserve the right to charge any patients that chronically fail to keep their appointments.

What if I have a dental emergency?

Our office provides on-call service for emergencies, 24 hours a day, 7 days a week, for all patients of record. If you have an after-hours emergency, you may call (763) 427-1311 and listen for the prompts. A message will be taken, and your medical/dental file will be given to Dr. Moffitt, who will return your call the same day. If your question is non-medical, a qualified staff member will be pleased to assist you. There is an emergency fee for after-hours care.

How do you handle insurance?

We are happy to assist you with filing your insurance claims. All insurances will be filed as out-of-network. All deductibles and co-payments must be paid at the time of service.

Note: If we cannot confirm patient eligibility and complete insurance information, the full amount is payable by the patient/account holder.

We will collect a fifty percent co-payment the day of service for all services that exceed $1,000.00 regardless of insurance coverage. The remainder of the balance is due upon completion of treatment. Once insurance has been processed, any overpayment will be mailed to you within one week of receipt of the insurance payment.

We will send you a statement once the insurance has been received. Most insurance companies will respond to your claim within four to six weeks. Please call our office if your statement does not reflect your insurance payment. The remaining balance after your insurance has been processed is your responsibility. Your prompt remittance is appreciated.

We recommend you get a prior authorization with your insurance company. A prior authorization can take 4-6 weeks for your insurance company to process. Once the prior authorization is filed, our office will expect the patient responsibility portion to be paid on the day of the treatment. Prior authorizations are estimates and are not always exact. Adjustments will be made if any over or underpayment occurs.

Finance charges will accrue at 1.5% per month, $2.50 minimum, on balances over 60 days old.

What if I do not have insurance?

Payment for treatment is due at the time of service. We accept cash, checks, Visa, and MasterCard, as well as CareCredit.

We will collect a 50% payment on the day of service for all services that exceed $1,000.00. The remainder of the balance is due upon completion of treatment.

A 5% cash discount will be applied to all payments received in full on the day of service with cash or a check. Senior citizens are eligible for a 10% cash discount if payment in full is received the day of service with cash or a check, up to $1000.00.

What is a Crown?

When decay is more extensive, we recommend a crown (cap). The crown offers you the strength, beauty, translucency, and feel of a natural tooth.

What are Inlays?

Defective or unsightly “fillings” can be replaced by all-ceramic inlays…an exciting tooth-colored material that is bonded to the tooth for added support. Instead of simply being held in by cement, this bonding process can improve the strength and beauty of your tooth.

What is a Veneer?

If you are not happy with the alignment, color, or form of your teeth, you can significantly improve your smile with ceramic veneers. A veneer is a thin layer of porcelain bonded to the front of your teeth and becomes part of the tooth. Typically, for optimal esthetics, multiple teeth may need to be veneered.

Is tooth whitening an option for me?

Tooth whitening is a process that lightens discolored enamel and dentin. Studies have shown that the proper use of dentist-monitored whitening systems effectively enhances your smile. Almost anyone can benefit from tooth whitening. Your prosthodontist can determine if you are a good candidate for home whitening through a thorough oral examination and diagnosis.
